- Policy Holder Full name
- Incident Date
- Elephant Policy Number

- Then click on Claim option.
- Now click on File a Claim online.
- There you will find two option A Glass Only Claim and All Other Claims. Select that applies to your order.
- Then you have to enter your personal information.
- Click on Next button.
