- Enter the 16-digit survey code from of your receipt. (An image of an example receipt will be there indicating the code)
- If you have the 16-digit code, click the link provided on the page.
- Click start to proceed.
- Answer the questions, rate your satisfaction, and tell your experience truthfully.
- Provide required details to enter the sweepstakes.
- Submit and finish the survey.
After completion, follow the instructions on the screen to enter the sweepstake for your chance to win the $500 check.
