- You will be asked to get an adult disability checklist, then you will be asked to complete the ‘Disability benefits application’, in red.
- You will be taken to a new tab, and here, you have to agree to the terms and services.
- Then, click on, ‘Next’.
- On the next page, you have to scroll down, and you will get the button for, ‘start a new application’ at the left.
- In the next, you have to confirm, if you are applying for yourself or you are helping someone else.
- Specify if you have my social account or not.
- Then, click on, ‘Next’.
- On the next page, you will get the online login page.
- If you don’t have an online account, then, you have to create an account.
Create an account for my social account
For this, you need to go to, www.socialsecurity.gov/applyfordisability

To create the account, you need,
- You must have a valid email address;
- Has to have a Social Security number;
- Must have a U.S. mailing address; and
- You have to be at least 18 years of age.
On the other hand, you cannot create an account for another person, if,
- With a person you have a business relationship;
- You are a representative payee of the person
- For the person, you are an appointed representative.
Then, you have to agree to the terms and services
Then, click on, ‘Next’, from the bottom.
On the next page, you need to enter,

- Last
- Suffix
- Social Security Number (SSN)
- Date of Birth

- City/Town
- State/Territory

- Email Address
- Confirm Email Address
- Specify would you like to request an upgrade code now?
- Then, click on, ‘Next’.
You have to follow the prompts after this and you will be able to create an account.
Logging into the account for my social account

Forgot username or password
If you have lost the login details of my social account, then you need to go to the same page login box, here, under the username at the left side you will get the option for, ‘Forgot username?’.
Here you have to input
- Email Address
- Social Security Number
- Date of Birth
- After typing this you have to click on, ‘Next’.
For Password, click on ‘Forgot password?’, for this, you have to enter,
- Email Address
- Social Security Number
- Date of Birth
- After typing this you have to click on, ‘Next’.

- Here, you have to scroll down to the bottom, and here, you will get three options, and you have to press on the third one, ‘Check application status’ in blue.
