- If you are a Purchaser at any of the Value Village stores, after purchase you will receive one receipt on which invitation of the survey will be printed along with a 19-digit survey code.
- Then Select “Receipt From Purchase or Exchange/Return” from the left bottom side of the survey page.

Enter the 19 digit survey code and click “Start” to begin the survey.

And if you have the “Donor invitation card” select the option from the right middle side of the survey page.

Then, you will be having the following information printed in your card –

4 digit Customer Code

Date of visit

Time

Then click on the “Start” button.

Now, you have to answer all the survey questions based on your last experience.

After completion, you will get a 5 Digit Thank you code.
You will get a discount of $2 on a purchase of $5 or more on your next purchase by using this 5 digit thank you code. Each receipt will be valid for one online survey only. Entry limit for each household for each month will be one.
